An ANSI Z535 sign audit is a structured facility walkthrough in which a safety team evaluates every hazard sign against the Z535 series criteria for signal words, message content, symbols, legibility, placement, and physical condition, then records each gap and assigns it a corrective action and an owner.

Most facilities do not have a signage problem. They have a signage drift problem. Signs get added one at a time, by different people, across years and shift changes. Nobody rereads the wall. The result is a floor where three different signal words describe the same class of hazard, a label too faded to read at the distance where it would still have mattered, and a warning bolted next to a machine decommissioned four years ago.
Drift is invisible until an auditor, an insurer, or an incident investigator makes it visible. This checklist gives you a repeatable way to find it first. Work it once with a clipboard, a camera, and a tape measure, and you will finish with a defensible record of what you have, what is wrong, and what to fix in what order.

For a facility walkthrough, Z535.2 does most of the work. If your organization also builds equipment that leaves the site, Z535.4 and Z535.6 come into scope and should be audited by the product team rather than the facility team.
The series runs on a roughly five-year revision cycle. Z535.2, Z535.4, and Z535.6 were revised in 2023, and Z535.1, Z535.3, and Z535.5 in 2022. If your reference copies predate those editions, replace them before you audit them.
2. Pull your existing sign register. If you do not have one, this audit creates it. That alone justifies the walkthrough.
3. Bring a camera and a tape measure. Photograph every deficiency in place. A photo of a faded sign next to the machine it guards is worth more in a corrective action review than any note you will write.
4. Bring someone from operations. You are auditing whether signs match hazards. Only the people who run the equipment know what the hazards are right now.
Step 1: Inventory before you judge
Walk the full route and log every sign before evaluating any. Mixing inventory with assessment is how audits stall halfway through building three.
For each sign, capture location, the hazard it addresses, signal word, format (stock or custom), material, and approximate installation date if known.
The most common finding at this stage is not a badly designed sign. It is an orphaned sign, one that refers to equipment, a process, or a chemical no longer present. Orphaned signs actively degrade the credibility of every other sign on the floor, because they teach workers that the signage is not maintained.

Step 2: Audit the signal word tier
For every sign, ask whether the signal word matches the actual severity of the hazard.
- DANGER, white text on a safety red panel. Reserved for hazards that will result in death or serious injury if not avoided.
- WARNING, black text on a safety orange panel. Hazards that could result in death or serious injury.
- CAUTION, black text on a safety yellow panel. Hazards that could result in minor or moderate injury.
- NOTICE, white text on a safety blue panel. Property damage, procedural, or practice information with no personal injury hazard.
- Is the symbol one your workforce recognizes? Comprehension testing is the only real answer here, and Z535.3 addresses evaluation criteria.
- Is the same symbol used for the same hazard everywhere on site?
- Where GHS pictograms appear alongside Z535 signal word panels, is the pairing consistent and non-contradictory?
- Is the sign at the point of hazard exposure, or merely somewhere near it?
- Is it within the normal line of sight, or above it where nobody looks?
- Is mounting height consistent across the facility?
- Is it obstructed by racking, equipment, seasonal storage, or a parked forklift? Audit this during working hours, not a quiet shift.
- For temporary hazards, are tags and barricade tape being used rather than permanent signage pressed into temporary service?
- Fading and UV degradation. Compare outdoor signs against an indoor sign of the same age. Red fades first, which means DANGER panels degrade toward an ambiguous pink before anything else does.
- Adhesion. Lifting corners on labels in washdown, high humidity, or temperature cycling environments.
- Abrasion and chemical attack. Especially near solvent handling and cleaning stations.
- Substrate mismatch. Vinyl where aluminum or a high durability plastic was needed for the environment.
- Does signage reflect the languages actually spoken on the floor today, not the languages spoken when the signs were ordered?
- Where bilingual signs are used, is the treatment consistent, and does neither language crowd out legibility?
- Have you asked workers what a given sign means? A comprehension spot check across a shift takes twenty minutes and tends to produce the most useful finding in the entire audit.

Two failure patterns to look for specifically:
Signal word inflation. DANGER used on hazards that do not carry a likely fatal or serious injury outcome. Inflation is the most damaging of the two failures, because it flattens the hierarchy. When everything is a DANGER, nothing is.
Signal word inconsistency across sites. If the same pinch point hazard reads CAUTION in one plant and WARNING in another, you have a standardization gap that will surface the moment anyone compares the two.
Record every mismatch alongside the hazard description, so the correction is defensible later.
Step 3: Audit the message panel
A functioning hazard sign answers three questions. Check each sign against all three.
- What is the hazard? Named specifically, not generically. "High voltage" is a hazard. "Danger" alone is not.
- What happens if it is not avoided? The consequence stated plainly.
- How do you avoid it? The required action or behavior.
Signs that state only the hazard are the most common shortfall in older facilities. They alert without instructing, which leaves the worker to supply the response themselves.
Check the writing itself as well. Message panel text should be headline style, active voice, and set in upper and lower case rather than all capitals, since mixed case reads faster at distance.
Step 4: Audit symbols and pictograms
Symbols carry the message for workers who cannot read the text, whether because of language, literacy, or distance.
Flag any symbol that appears in only one location. One-off symbols are almost always legacy artifacts.
Step 5: Audit legibility and viewing distance
This is the step most audits skip, and it is the one that determines whether a sign works at all.
The governing principle is that text must be readable by a person with normal or corrected vision at the safe viewing distance, meaning the closest point at which someone could still read the sign and respond in time to avoid the hazard. Measure that distance for each sign rather than assuming it.
No single letter height satisfies this everywhere, which is why the standards do not publish one. Required height is a function of that measured distance, plus lighting, background contrast, typeface, viewing angle, message complexity, and how much reaction time the hazard allows. Two identical signs can pass in one location and fail in another.
What to check against: the letter height and safe viewing distance criteria in your licensed copy of the applicable standard, Z535.2 for facility signs and Z535.4 for product labels. Record the measured distance for each sign on the worksheet so the judgment is reviewable later.
Practical field test: stand at the point where a worker would first need the information, not at the point where the sign is convenient to read. If you cannot read it there, it fails, whatever the specification says. This test catches problems that a tape measure and a table will not, including glare, obstruction, and signs mounted above the natural line of sight.

Then set the re-audit interval. Annual review is a common cadence, with additional triggers on any process change, equipment change, chemical introduction, facility modification, or incident involving a hazard that had signage.
Retain the completed checklist, the photographs, and the corrective action record together. That package is what demonstrates a maintained program rather than a one-time cleanup.
